Nominate an Extraordinary Person to be the Medfield Foundation's Volunteer of the Year or Youth Volunteer of the Year
The Medfield Foundation chooses both its Medfield Foundation Volunteer of the Year
and its Medfield Foundation Youth Volunteer of the Year in the spring each year from amongst
the people nominated during the year by the public. Any one can nominate extraordinary
individuals for the Medfield Foundation volunteer of the year recognition by completing the
information below.
The Medfield Foundation makes its choice from amongst the nominees based solely upon
the materials submitted with the nominations. The Medfield Foundation itself does not do any
additional independent data gathering on nominees and the Medfield Foundation board does not
rely upon its own knowledge of the nominees beyond what appears in the nominations.
Therefore, if you are nominating a person, please be sure that the reasons that your nominee is
deserving are fully stated. If there are additional specific things that you would like the Medfield
Foundation to consider beyond what you have already written, please feel free to supplement
what you have already submitted or to have others provide additional information.
In the spring, when the Medfield Foundation recognizes all the extraordinary volunteers at an
event to which you and they will be invited, it will be helpful to have contact information for
your nominee (address, telephone and email) so that they can be invited.
Thank you for taking the time to highlight someone in Medfield deserving of special recognition
for his or her volunteer work. The Medfield Foundation believes that your taking the time to
make your nomination and writing about this extraordinary individual's accomplishments has
actually provided public recognition of this extraordinary person, which action has already made
