Medfield Volunteer Awards

Nominate extraordinary residents for their service to Medfield

Annually since 2008, the Medfield Foundation has recognized the stellar community service efforts of its residents through the Medfield Foundation Volunteer of the Year award. Today, the Foundation has three awards that honor individuals whose extraordinary efforts and activities have made a marked difference to the quality of life in Medfield:

  • Volunteer of the Year
  • Youth Volunteer of the Year, and
  • Lifetime Achievement (NEW - for a volunteer who has dedicated many years to a town cause)

To nominate a Medfield resident for one of the awards above, please download and submit the official 2012 Volunteer Nomination Form.

Here are the steps to take:

  1. Save the Word 2007 file to your computer and make sure it's trouble-free (if you have problems, please e-mail the committee ASAP at mfivolunteeraward@gmail.com).
  2. Review the nomination criteria on the first page and subsequent questions to determine what information you may need to collect from the volunteer you are nominating (you may also want to alert your reference in advance that he/she will be contacted)
  3. Fill out the form (and be sure to save it as you go; you do not need to fill out the form in one sitting).
  4. When you have completed all the questions, save the file one last time to your computer as either a Word 2003 or 2007 document or a PDF (recommended, but either is fine). You may also want to print a hard copy.
  5. Attach the file to an e-mail and send it to mfivolunteeraward@gmail.com no later than Feb. 10, 2012.

Basic requirements for all awards:

  • Nominee must be a Medfield resident (but nominators are not required to be Medfield residents).
  • Nominee's community service must have been performed without compensation (though in rare instances exceptions may be granted).
  • Nominee's volunteer work must have occurred in Medfield to support a community organization, initiative or cause.
  • Nominations are limited to ONE per nominator, per category. For example, the same person can nominate someone for the Youth Volunteer award, as well as someone for the Volunteer of the Year award and/or Lifetime Achievement award. (A new nomination form must be filled out for each.)

General nomination criteria for all awards:

  • Action -- What does the nominee actually do in his/her capacity as a volunteer?
  • Need -- What community need(s) does the nominee address?
  • Impact -- How is nominee's volunteer work making a difference?
  • Inspiration -- What makes the nominee unique or special?
  • Verification -- Who can vouch for the nominee's volunteer commitment and accomplishments?

Decisions on each award are made solely based on the materials submitted by the individuals making the nominations, and the information shared by the nominator's reference. The Medfield Foundation does not collect any additional background information or make award selections based on its own relationships with those being nominated. To help ensure an objective process, this year judges from within the community, past award recipients and the nonprofit field will help to evaluate the nominations and determine the three primary 2012 award recipients. At the annual event, additional volunteers will also be recognized.

All nominees, the individuals who nominated them, and members of the community are invited to attend the 2012 Medfield Foundation Volunteer Award Event that will be held the afternoon of Sunday, March 18 2012 at the Center at Medfield on Ice House Rd.
